Hand in Hand believes in the importance of looking at the big picture when working with each client. We complete a medical evaluation and review it with family, discuss the client’s personality and preferences, and assess his or her unique financial situation in order to make suggestions for care options.
Review Medical Needs
Hand in Hand’s medical assessment helps to rule out communities that can not manage the medical needs of your loved one. This assessment can be completed in the comfort of your own home, at our business office, or at any area health care facility.
In addition to this medical assessment, clients interested in personal care homes that are located in Pennsylvania must have his or her physician complete a Documentation of Medical Evaluation (DME) form.
Discuss Options
In addition to discussing the unique medical needs of your loved one, our Care Coordinators will also ask questions pertaining to the social and financial background of your loved one. This information helps Hand in Hand drill down to options that are affordable and appropriate for each unique situation.

Schedule Tours
Hand in Hand will ensure that all arrangements and documentation are completed prior to move-in. Individual community requirements are reviewed with each family prior to admission, and, as appropriate, forms are submitted to the physician and community of choice.
